I understand that CHSC will collect and store information pertaining to my health, my personal contact details and other necessary information for the purposes of conducting medical screening services (“Personal Information”). I consent to CHSC and its affiliates, using the Personal Information, and sharing the same, (including sharing access to my electronic medical record through secure electronic means) with the following parties/entities for the stated purposes:

a) other healthcare providers and laboratories for the purposes of patient care including the sharing of Personal Information for tele-health related purposes.

b) any contracted third parties, agents, consultants for the purpose of conducting de-identified research and analysis and conducting general health and wellness programs.

c) for meeting legal and statutory obligations including disclosures to regulatory authorities like the Department of Health.

d) third party business affiliates within UAE, solely for the purposes of either:
(i) assessing CHSC’s clinical performance or
(ii) CHSC’s patient satisfaction levels.

All credit/debit card details and personally identifiable information will NOT be stored, sold, shared, rented, or leased to any third parties.
